Tumor marker questions are some of the most reliably one-to-one in Pathology — a marker, a cancer, done. The trap isn’t the matching; it’s assuming these markers are used for screening the way people assume, when most of them are really monitoring tools.

The Distinction Examiners Actually Test
Most tumor markers lack the sensitivity and specificity to be useful screening tools in an asymptomatic population — they’re elevated by plenty of benign conditions too (CA-125 rises in endometriosis and even menstruation; CEA rises in smokers and inflammatory bowel disease). Where they earn their place clinically is in monitoring: tracking a marker after treatment to catch recurrence early, or watching it trend down to confirm treatment response.
PSA is the frequent exception raised in screening discussions, but even there, current practice treats population-wide screening as a nuanced, patient-specific decision rather than a blanket recommendation — a detail worth remembering if a question frames PSA as a universal screening tool.
A Few Pairings Worth Double-Checking
- AFP is elevated in hepatocellular carcinoma and yolk sac tumors, but not in pure seminoma — a classic distractor, since seminomas can still show mild β-hCG elevation.
- CA 19-9 is the go-to for pancreatic cancer, but it’s also elevated in cholangiocarcinoma and benign biliary obstruction, so it isn’t purely cancer-specific either.
- Alkaline phosphatase rising with bone metastases is a liver-independent clue — pair it with a normal GGT to confirm the source is bone rather than liver.
